LPN - Resident Care Coordinator

OVERVIEW:
The LPN/Resident Care Coordinator is responsible for overseeing all aspects of resident care, including, but not limited to, the following responsibilities:

RESPONSIBILITIES:

  • Conduct self in calm, pleasant manner, promote positive, professional image.
  • Work effectively and respectfully with all members of team and corporation, residents and families, members of the medical community.
  • Consistently punctual, reliable, dependable and willing to accept new challenges.
  • Able to accept direction and guidance from supervisors, able to maintain effective working relationships with other team members.
  • Medication pass according to facility protocol and all aspects of medication management.
  • Implementing resident treatments and procedures as ordered by physician.
  • Coordinating resident care with outside resources i.e. physicians, labs, testing, etc.
  • Possess strong assessment, documentation, communication and interpersonal skills.
  • Complete initial and follow up resident assessments, as well as, MA51s and state screeners in conjunction with the Assistant Director.
  • Assist resident aide staff as needed with direct care, activities, and van transportation.
  • Maintain current nursing licensure and CPR certification.
  • Effectively assist with training and supervision of resident aide staff and serve as professional resource.
  • Serve as mentor/trainer for new LPN staff.
  • Provide facility tours, complete admission process and contracts autonomously (as necessary) in conjunction with Assistant Director and Director.
  • Practice safety procedures and comply with established reporting of incidents and injuries to employees and/or residents.
  • Complete all orientation requirements, participate in orientation of resident aide staff and mandatory staff meetings and inservices.
  • Knowledgeable of state regulations.
  • In conjunction with management team, assist in developing and maintaining an environment that ensures the health, safety, welfare, and independence of our residents and provides a continuum of quality care.

REQUIRED SKILL, EDUCATION, EXPERIENCE:

  • Completion of accredited Practical Nursing Program with current Pennsylvania licensure in good standing.
  • Current CPR certification.
  • Prior experience in providing health care to senior population. Experience in assisting living preferred but not necessary.
  • Must have current Pennsylvania driver's license and automobile insurance.

PHYSICAL, MENTAL AND ENVIRONMENTAL REQUIRMENTS:

  • Ability to lift and transfer residents. Able to bend, reach, push, pull, stoop, twist, lift, walk, stand and sit.
  • Requires alertness, attention to detail, effective time management skills and ability to multi-task. Must be able to prioritize, use good judgment and make sound decisions. Maintain positive and professional manner.
  • Work in well lit, clean, cooled and heated areas.
  • Potential for injury from aggressive, irrational resident or work related incident.
  • Potential for exposure to infectious agents or hazardous chemicals.
  • Able to work overtime as required to meet the needs of the facility.
